FMLA

Basic Leave Entitlement
FMLA requires covered employers to provide up to 12 weeks of unpaid,
job-protected leave to eligible employees for the following reasons:
• for incapacity due to pregnancy, prenatal medical care or child birth;
• to care for the employee’s child after birth, or placement for adoption
or foster care;
• to care for the employee’s spouse, son, daughter or parent, who has
a serious health condition; or
• for a serious health condition that makes the employee unable to
perform the employee’s job.

Please contact Myra Simmons (720-913-3408) or Nicole Mondragon (720-913-3453) at least  30 days in advance to apply for FMLA unless it is an emergency situation. They will send the paperwork to you via email, U.S mail, Department mail, or fax. Once the paperwork is completed and returned to Myra or Nicole and approved by them then the member can contact Admin and use vacation days, sick leave or LWOP for FMLA.
