To use this form, fill out the editable fields then click the download button and select "With your changes" to download the completed form. Once downloaded follow the instructions on the form for submission.

Important Information

• Badges may only be purchased by Retired or Active Denver Fire Members
• Members may purchase badges for current or previously held ranks
• All purchased badges will be sent to DFD Headquarters for inspection before being released to the purchaser
• Prices are subject to change after May 30th, 2027
