Benefits Plan Dependent Audit Underway
All employees who cover dependents have received a letter at their home address to verify their dependents being covered under the Benefit plan.
A dependent audit is verification that the dependent(s) included on an employee’s benefits are eligible for coverage.
Please respond to the letter per the instructions as soon as possible. If you cover dependent(s) on the city’s group benefits (medical, dental, vision, and/or supplemental life).

Go to: https://app.unifyhr.com
Login Name: Enter your 6 digit Payroll ID-(The Payroll ID is listed next to your F# in Telestaff)
Temporary Password: This is your date of birth (format mmddyy) and the last four digits of your SSN
Example: DOB is 09/24/70 and the last four digits of SSN are 8587- Password =0924708587
You will be instructed to change your password upon entering the secured site.
Follow the directions on uploading your documents.
