FMLA requires covered employers to provide up to 12 weeks of unpaid, job-protected leave to eligible employees which include:

      • Incapacity due to pregnancy, prenatal medical care or child birth
      • Care for the employee’s child after birth, or placement for adoption or foster care
      • Care for the employee’s spouse, son, daughter or parent, who has a serious health condition
      • A serious health condition (intermittent/continuous) that makes the employee unable to perform the employee’s job

Please contact Safety HR Leave Team at least  30 days in advance to apply for FMLA (unless it is an emergency situation) and they will send paperwork to you via email.

Once the paperwork is completed by the treating Physician and returned to Safety HR Leave Team via email/fax the member will then work with Safety HR Leave Team and  Administration/HR on how their time will be used i.e. vacation, sick leave, or LWOP for FMLA.
